At ACHIEVE clients come to us with or without a physician's referral. In Nebraska you can receive physical therapy services without orders or a prescription. We hold great pride in the fact that more than 90% of our clients come to us from a friend or family member who was a client.

For your first visit please download the form(s) above or arrive 15 minutes early to complete the necessary paperwork. Remember to bring your insurance card and any other important information. Please wear or bring with you gym type loose clothing.

Your first session will last approximately one hour. We will review your medical history and an initial examination will be performed.


Important insurance information on direct access:

*While Nebraska is a "direct access" state, some insurance companies and/or policies require that a client have orders or a prescription from a medical doctor in order to pay for PT services. Here at ACHIEVE we make this easy for you. Give us a call and we will be happy to contact your insurance on your behalf and help you with the necessary steps they require.

Brian has an extensive background in biomechanics, athletics, sports training and rehabilitation. As a graduate of Creighton University, Brian earned his Doctor of Physical Therapy degree with Honors.

He is one of the first in the country to be certified as a a Muscle Activation Techniques Specialist Mastery Level​ and Resistance Training Specialist Mastery Level.  These certifications make him the first and only in Nebraska and Iowa certified as such. Additional certificates include; CrossFit Level 2 Trainer, Certified Strength and Conditioning Specialist, Personal Trainer, Ignite360 Trainer, and Ergonomic Evaluation Specialist.

Utilizing his skills and experience to enhance recovery time and level of play; Brian works with local high schools, colleges, professional and semi-professional athletes. As a former collegiate football player; Brian has a full understanding of the demands placed on a student/athlete and knows what it takes to be successful.  Brian enjoys spending time with his wife Erin, and their 6 children: Brianne, Camerin, Colt, Ayden, Danni and Ashtyn.

Katherine attended graduate school at Bellarmine University where she earned her Doctorate in Physical Therapy. She has been trained in functional dry needling techniques through the Myopain Seminar and is a Muscle Activation Technique Specialist.  Kat has her Crossfit Level 2 certification and an extensive background in sports and training. Kat earned a degree in Medicine, Health, and Society at Vanderbilt University, which she attended on a track and field scholarship for pole vaulting. During her collegiate career she made the SEC honor roll, qualified for the track and field East Regionals, and held the Vanderbilt Indoor and Outdoor school record in the pole vault. In high school, Kat participated in gymnastics, track and field, dance, wrestling, and cheerleading. She was 3x Illinois State champion in the pole vault and state record holder for Class A. Recently, Kat has been competitively involved in Crossfit qualifying for the Central regional as an individual in 2013 and as part of a team in 2017. She enjoys spending time with friends and family, competing against her husband, Brandon, in crossfit workouts, and watching Netflix.
